I, [swift_name size="medium"], and Watch My Wag agree to the following:

I certify that my animal is/will be up to date on all shots including Rabies, Distemper and Bordetella prior to entering the WMW facility. I will deliver vaccinations via EMAIL or PAPER COPY upon arrival.

I understand my animal will not have services provided if they are not properly vaccinated.

1. Watch My Wag agrees to use reasonable care, to keep the facilities of Watch My Wag (the “Facility”) clean and properly secured, and to not remove any pet from the Facility except as described in number 3 below.
2. The Owner, in exchange for and in consideration of the services provided by Watch My Wag, Watch My Wag (the “Services”), hereby RELEASES, WAIVES, DISCHARGES, AND COVENANTS NOT TO SUE Watch My Wag, its divisions or related or affiliated entities, or their officers including without limitation, boarding, grooming, and related activities managed or controlled by, managers, members, representatives, servants, agents, successors, heirs or employees (the “Releasees”) from and against any and all liability, claims (including outstanding claims), demands, actions, and causes of action whatsoever arising out of or related to any loss or damage that may be sustained by the Owner’s pet (the “Pet”) or other property from disease; death; escape; theft; fire; injury to persons, other pets, or property by the Pet; or other unavoidable causes, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES or otherwise.
3. If Watch My Wag discovers that the Pet admitted for grooming services has fleas, ticks, and/or other similar infestations, the Pet shall be treated at the Owner’s expense. If the Pet suffers an injury or becomes ill, Watch My Wag will use its best efforts to notify the Owner promptly. If the Owner does not immediately inform Watch My Wag of the actions the Owner wishes to be taken, or if the injury or illness of the Pet, in the sole discretion of Watch My Wag, reasonably requires swift action, Watch My Wag shall have the right to bring the Pet to a veterinarian, which Watch My Wag shall reasonably choose in its sole discretion, or administer medicine or other appropriate treatments, which Watch My Wag shall reasonably administer in its sole discretion. Reasonable expenses resulting from such actions shall promptly be paid by the Owner prior to the Pet’s release from the Facility.ge for and in consideration of the services provided by Watch My Wag, including without limitation, boarding, grooming, and related activities managed or controlled by Watch My Wag (the “Services”), hereby RELEASES, WAIVES, DISCHARGES, AND COVENANTS NOT TO SUE Watch My Wag, its divisions or related or affiliated entities, or their officers, managers, members, representatives, servants, agents, successors, heirs or employees (the “Releasees”) from and against any and all liability, claims (including outstanding claims), demands, actions, and causes of action whatsoever arising out of or related to any loss or damage that may be sustained by the Owner’s pet (the “Pet”) or other property from disease; death; escape; theft; fire; injury to persons, other pets, or property by the Pet; or other unavoidable causes, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES or otherwise.

4. Unless the Owner indicates below, dogs will be permitted to interact with other dogs in unsupervised groups of up to four dogs or supervised groups of up to ten dogs. Watch My Wag shall keep all dogs in groups of reasonably similar size and disposition. The Owner acknowledges and understands the risk and potential liability created when two or more dogs interact. The Owner hereby accepts all responsibility for any injuries and/or damage caused or sustained by the Owner’s dog.

5. I DO NOT WANT MY DOG TO HAVE CONTACT WITH OTHER DOGS
   Unless the Owner indicates below, the Owner authorizes Watch My Wag to take and use photos of the Owner’s dog(s) in advertising, social media, and other marketing materials.

1. Unless arranged otherwise by Watch My Wag and the Owner, amounts owed by the Owner for all services provided by Watch My Wag, which may include, without limitation, boarding, grooming, training, medicine, or veterinary services, shall be due and payable by the Owner prior to the Pet’s release from the Facility. If the Pet is not retrieved within ten days of the scheduled discharge date, Watch My Wag shall send notice of intended rehoming of the Pet by registered mail to the Owner at the address given below and no further notice shall be deemed necessary for Watch My Wag to rehome the pet. Watch My Wag shall have the right to rehome said pet within ten days of such notice.
2. Any deficiency in payment is due and immediately payable by the Owner upon notice by Watch My Wag. If the pet causes any damage to Watch My Wag property, the Owner shall be responsible for the cost of replacing or repairing such property, and any amount owed by the Owner to replace or repair such property is due and immediately payable by the Owner upon notice by Watch My Wag.
3. I am aware that the hours of operation for pick up and drop off are Monday-Friday 7a-12p and 2p-6p (Excluding Holidays) and Saturday-Sunday 8a-10a and 2p-4p (Excluding Holidays). I understand if I do not arrive before closing time my dog will be kept until the facilities next scheduled hours of operation unless prior arrangements have been made.
4. I HAVE REVIEWED THE NEW PRICES EFFECTIVE 5/26/16 POSTED ON WATCHMYWAG.COM INCLUDING TRAINING, DAY PLAY, GROOMING, AND BOARDING.
